Job Details
CrossNation Aged Care Partner
| Location | Victoria |
| Employment | Full-Time, Ongoing |
| Team Focus | Aged care, Support at Home |
| Start | As soon as possible |
| Positions | Multiple Care Manager positions across Victoria and South Australia |
Role Snapshot
CrossNation Aged Care Partner is recruiting Care Managers, also known as Case Managers, to support older Australians who receive care at home under the Support at Home program. Support at Home replaced Home Care Packages on 1 November 2025.
As a Care Manager you will look after a caseload of clients. You will work with each person to plan, coordinate and review the services they receive, so they can keep living safely and independently at home. This opportunity is based in Victoria.
You do not need previous Support at Home or Home Care Package experience. A comprehensive training program covers the organisation's systems, its processes and the Support at Home program, so strong people skills and a background in care, coordination or case management matter more than program knowledge.
